Min block/damage gear fixes and end game scaling

Gear that reduced min block or min damage had their penalty applied twice... should be fixed now :) Also redused end game enemy scaling a bit.

Jewel neck nerf & general fixes

Jewel necklace now only gives 45 max coin gain instead of 65. Leaderboard code cleaned up, possible crash fix on stratup. Game uses fullscreen window now, instead of exclusive fullscreen, should fix problems with alt tabbing back and fourth. Alberto has custom dialogue. Other minor fixes.
